Owner Surrender Form

Please realize ECGGR cannot accept Mastiff's from all over the country. We concentrate on helping Mastiff's in parts of the following states: MA, MI, NY, NJ, DE, MD, D.C., WV, VA, NC, SC, GA, FL. We also do not have volunteers in every state, therefore, if you are outside of a 2 hour driving radius it will be your responsibility to arrange transportation for your dog to us. ECGGR is a purebred rescue and as such, we are sorry to say we cannot take Mastiff mixes.  If the dog in your possession is not a purebred Mastiff please contact a local mixed breed rescue.

Your dog must be current on vaccines, spayed/neutered, heart worm negative and on preventative and micro chipped before coming into rescue. The owner will have the option to make a donation to ECGGR to help us recover some of the cost of this health care provided if the dog has no medical care completed. If you are unable to meet these prerequisites, please explain more about your situation here then complete the rest of this form. ECGGR will consider your circumstances, and a decision will be made to determine if your dog will be accepted into ECGGR. It may take several days for us to get back to you with the results of this decision, please be patient. If any questions are left un-answered, this form will NOT be considered;

Dogs that are NOT current on vaccines require a $250 donation
Dogs that are NOT current on vaccines or spayed/neutered require a $400 donation
Dogs with a MAJOR medical issue (surgery need, heart worm positive, senior, broken leg, ccl repair, pregnancy, pyo) require a MINIMUM of a $800 basic care donation for food expense, flea/tick and heart worm med. expense and joint supplements and initial vetting. 
This donation is made at the time of the surrender and in cash only. 

You are also requested to submit pictures of your dog, preferably a front and side view to determine that the dog is definitely a Mastiff. If you have more than one dog to surrender, please fill out one application for each dog, making sure your name and email address is on each application.
